Household Application for Free/Reduced Meals
If you feel your family
may qualify for free / reduced meals, please fill out an
application and turn it into the office Applications are
available in the office, the Child Nutrition Office, or the
Administration building. You are welcome to fill out an
application any time throughout the year if you've had a change
in income, household size, etc., that might qualify you family
for free / reduced meals.
A new application on the correct year's form must be
filled out each year in order to qualify for the program.
Please do not fill out separate application for each child.
List all children in the same household on one form.
